Printing the 2 pages on both sides of 1 piece of paper is a great idea and helps preserve the environment for our future generations to enjoy! Once you have entered the required information, click the "Generate BOL" button - a new window will open with your ready to print BOL. Use the "File -> Print..." option from your navigator menu to print the BOL. Before shipping, be sure to sign and initial where indicated.

Request for Cargo Insurance Click here to open our cargo insurance request form (requires Adobe Acrobat Reader). In the fields indicated, please include the value of the insured item and the freight charges. UTS will fill in the fields pertaining to Ports of lading/discharge, carrier, vessel and voyage, and house bill/master bill. Once you have completed and printed the form, fax it to UTS at (818) 843 - 2514.	Damage/Loss Claim Form (PDF) We hope you will have little need to use the damage/loss claim form; however, Murphy's Law dictates that sometimes things can go askew. Print out and complete our claim form. (requires Adobe Acrobat Reader). Once you have completed and signed the form, please fax all required documents to UTS [(818) 843-2514]; ensure you have all documentation required (see items 1-5 on claim form) to help us quickly process your claim.
